Na sua Seção IV - CARACTERIZAÇÃO DO ESTABELECIMENTO / LOCAL ALVO DO PROCEDIMENTO, o formulário define os termos Albergante e Albergado repetindo (ou dando origem à) (a) definição no dicionário informal mas também exemplificando a definição da seguinte maneira.
