I think For. ansv. is short for foreldreansvar, which in English is parental responsibility according to Kunnskapsforlagets NO-EN dictionary.

Fødekomm. means county of birth (fødekommune)

Let me explain what is meant by "Fødekomm/land:".
People born in Norway should enter their Norwegian local council of birth while people born in other countries should enter their country of birth.

About "Fødekomm/land":
It could also be that people should fill out both their local council of birth and their country of birth, like for example: "Copenhagen, Denmark".

agree EC Translate : As it appears to be a case of a child either moving to or from Norway, the form asks about the nature of the parental responsibility, which in this regard seems to be shared. You can split a responsibility, but you share a child:o)
