Discussion

Michael Barnett Sep 12, 2010:
Monocular, bilateral? With reference to DrManu's suggestion, amaurosis fugax is by definition monocular, far too specific a term for this situation. Acces amaurotique is simply a bout of visual loss, without comment on whether it is bilateral or monocular.

bout of loss of vision

Amaurosis is simply a loss of vision. In this context, the cause is the encephalopathy.

Peer comment(s):

disagree Michael Barnett : Amaurosis fugax is a specific kind of amaurosis, not the sort described in the text. Amaurosis fugax refers to an occlusion of the central retinal artery.

What are the common signs and symptoms of Idiopathic Intracranial Hypertension?
Most patients with IIH experience headache. The headache may be aggravated by changes in position. There may be a ringing or a "whooshing" sound in the ear. A patient may experience double vision.

In this disorder, it is common to have momentary loss of vision known as "transient obscurations of vision". Other visual symptoms are a central blind spot, blurred vision, or loss of peripheral vision in one or both eyes. The visual loss may be mild to severe. If severe and untreated, it can result in blindness. The optic nerves are usually swollen in appearance.

Pathophysiology
Ischemia reduces delivery of oxygen and other important nutrients to tissues, causing metabolic compromise of cells. Functional deficit may be temporary or permanent, depending on the degree of damage. Nomenclature of eye ischemia as given by Hedges and others includes the following:1

Transient visual obscuration (TVO) - Episodes lasting seconds that are associated with papilledema and increased intracranial pressure
Amaurosis fugax - Brief, fleeting attack of monocular partial or total blindness that lasts seconds to minutes
