This sentence is part of a longer one which is not clear to me though the words are not difficult.
'The personal details contained in this certificate are those supplied by or on behalf of the person to whom the certificate relates at the time of the application was made and that appear to match any conviction or other details linked to that identity.'

Mark, Nicole, Oui, c'est possible (je n'avais pas noté l'en-tête de la question). Merci! Ça expliquerait le style de la phrase. Le cas échéant, il vaudrait peut-être mieux de se tenir au plus proche du texte original:
Les renseignements personnels qui figurent dans ce certificat sont ceux qui ont été fournis par la personne visée ou pour son compte au moment où la demande a été produite et qui semblent correspondre aux [condamnations] [antécédents judiciaires] ou autres renseignements se rattachant à cette identité.

Hi Carol, indeed it should read 'at the time the application was made', sorry for the typo. And like Phil, I think the English is badly written myself that is why I am seeking the help of colleagues.

conviction In the context of the source text, conviction means "condamnation" , "antécédents judiciaires".

Agree with Carol: the details were supplied by the person in question or by their agent/guardian/lawyer or someone else acting for them. This would happen if the person in question is under-age, illiterate or unable to speak the language, incapacitated in some way, or unable to attend for any reason.
