Any sophisticated way to create line fields and replicate the layout in Word (2019) as in the following document?

Underline (Ctrl+U) and TAB Nov 5, 2021

What I usually do is I press Ctrl+U and then TAB key to create a line, and then I put the cursor on the line and type the text I want, having checked that the text being typed in underlined.
