Source text - English The symptom of craving has been re-introduced to DSM-5 as one of the 11 criteria for alcohol use disorder (AUD). This revision to the diagnostic system is supported by a host of studies implicating craving in the phenomenology of AUD, including documenting the experience of craving in individuals seeking treatment for AUD (Oslin et al., 2009), association of craving with severity of AUD (MacKillop et al., 2010), craving as a predictor of relapse to drinking (Higley et al., 2011), and human laboratory investigations of craving during experimental provocation (Ray, 2011; Sinha, 2009). In light of the adoption of craving as a symptom of AUD, questions about the optimal assessment of craving in clinical populations have been raised, particularly given the lack of consensus on the development and maintenance of craving (Rosenberg, 2009).
Craving is inherently a subjective experience, marked by the strong desire to consume a given substance. As reviewed in detail elsewhere (Ray et al., 2013), there are a host of instruments psychometrically validated to measure craving for alcohol. Importantly, the assessment of craving is often divided into tonic and phasic states, with tonic describing a more stable and unproved experience of craving while phasic is marked by provocation and short duration of assessment. One of the most widely used assessments of tonic alcohol craving is the Penn Alcohol Craving Scale (PACS) which has demonstrated reliability and validity (Flannery et al., 2003; Flannery, Volpicelli, & Pettinati, 1999). As noted by Murphy et al. (2014), using a psychometrically validated measure of tonic craving such as the PACS can advance the diagnosis of alcohol craving.
Translation - English O sintoma da fissura foi reintroduzido ao DSM-5 como um dos 11 critérios diagnósticos para o transtorno por uso de álcool (TUA). Esta revisão do sistema de diagnóstico tem base em uma miríade de estudos que indicam a fissura na fenomenologia do TUA, incluindo documentação da experiência de fissura em indivíduos buscando tratamento para o TUA (Oslin et al., 2009), a associação da fissura com a severidade do TUA (MacKillop et al., 2010), a fissura como preditora de recaídas ao alcoolismo (Higley et al., 2011), e investigações laboratoriais da fissura em humanos durante provocações experimentais (Ray, 2011; Sinha, 2009). Source text - English Despite the increased emphasis on memory in recent decades, the
notion that the hippocampal–diencephalic–cingulate network
(Papez, 1937) is vital for emotion never fully disappeared. From
the 1950s to the 1980s, ideas about the limbic system remained
centred on its likely role in emotion and its presumed imbalance
in psychiatric conditions (Kelly, 1973; Livingston and Escobar,
1971). This preserved emphasis partly reflected how the term
‘limbic system’ had been broadened to include areas such as the
amygdala and orbitofrontal cortex. Reflecting this trend, it was
proposed that that the limbic system should be subdivided
(Livingston and Escobar, 1971; Rolls, 2015). One subsystem was
a ‘medial limbic’ circuit (Livingston and Escobar, 1971), which
particularly emphasised anterior thalamic–posterior cingulate–
hippocampal connections, that is, the connections highlighted in
this review. This ‘medial limbic’ circuit was contrasted with an
amygdala-based ‘basolateral circuit’ that included the anterior
cingulate region, the two circuits jointly contributing to affect and
learnt emotion (Dalgleish, 2004; Livingston and Escobar, 1971

Esta ênfase conservada refletia parcialmente a ampliação do termo "sistema límbico" para incluir áreas como a amígdala e o córtex orbitofrontal. Source text - Portuguese A depressão resistente ao tratamento (DRT), um transtorno depressivo de grande relevância entre os tipos conhecidos, é definida como uma falha em responder a dois ou mais ensaios de monoterapia com fármaco antidepressivo ou ainda uma ausência de resposta a quatro ou mais ensaios de diferentes terapias antidepressivas. Sua prevalência estimada é de 350 milhões de diagnósticos no mundo.
Ainda assim, existem medidas terapêuticas alternativas para pacientes diagnosticados com DRT, como, por exemplo, a terapia eletroconvulsiva (ECT, em inglês). Tal procedimento é baseado na indução de convulsões por meio de corrente elétrica, a fim de reorganizar as conexões neuronais do cérebro. Apesar de seguro e de apresentar resultados efetivos para diversos transtornos mentais, a ECT é associada a efeitos deletérios e a uma ação demorada.
Contudo, nos últimos anos, percebeu-se que um conhecido anestésico também poderia ter efeito antidepressivo, inclusive em casos resistentes. Trata-se da cetamina, descoberta na década de 1960, a princípio usada como anestésico e, em menor proporção, como analgésico. No entanto, anos depois, sua ação antagonista de receptores do glutamato/aspartato (receptores N-metil-D-aspartato ou NMDA) foi evidenciada, sendo o glutamato um importante neurotransmissor envolvido na modulação de variadas atividades cerebrais. Assim, a cetamina passou a ser alvo de diversas pesquisas para entender seu potencial de regulação neuronal para transtornos mentais e possibilitar outra alternativa ao tratamento da DRT.
Translation - English Treatment-resistant depression (TRD), a depressive disorder of great relevance among the known types, can be defined as unsuccessful response to two or more monotherapy trials with antidepressant drugs, or even as an absence of response to four or more trials with different antidepressant therapies. Its estimated prevalence is 350 million diagnoses worldwide.
Still, there are alternative therapeutic measures for patients diagnosed with TRD, such as electroconvulsive therapy (ECT). This procedure consists on the induction of seizures through an electric current, in order to rearrange the neural connections in the brain. Although safe and with proven effective results for various mental disorders, ECT is associated with harmful effects and delayed action.
However, in recent years, arose the realization that a known anesthetic could also have antidepressant effects, even for resistant cases. That is ketamine, discovered in the 1960s, initially used as an anesthetic and, to a lesser degree, as a painkiller. Nonetheless, years later, its glutamate/aspartate receptor antagonist action (N-Methyl-D-aspartate or NMDA receptors) was demonstrated, glutamate being an important neurotransmitter involved in the modulation of diverse brain activities. And so, ketamine became a central topic for researchers who strive to understand its potential for neural regulation in mental disorders and provide a new alternative for TRD treatment.

Source text - Portuguese Na investigação diagnóstica, o paciente foi submetido a testes genéticos para avaliação de neurofribromatose tipo 1 (NF1), os quais mostraram ausência de variantes patogênicos para os genes NF1, NF2 e SPRED1, porém paciente apresenta achados sugestivos de NF1 em ressonância magnética do encéfalo. Ao longo do acompanhamento com o geneticista foi encontrado uma lesão tumoral ventricular o qual passou a ser acompanhado com estudo radiológico periodicamente. Os exames seriados mostraram um aumento da lesão em um curto período, o que levou os familiares a buscarem avaliação neurocirúrgica. O achado radiológico era de uma lesão expansiva na topografia do lobo frontal à direita com invasão do corno anterior do ventrículo lateral em íntima proximidade com forame de monro, porém sem sinais de hidrocefalia. Diante do crescimento da lesão, sem diagnóstico da celularidade definido, foi decidido e realizado biópsia guiada por neuronavegação acompanhada de procedimento endoscópico com abertura do tubercinério. O achado anatomopatológico foi compatível com astrocitoma grau I, logo patologia benigna. Com diagnóstico de patologia benigna e diante do risco inerente a uma abordagem para ressecção da lesão, foi decidido apenas por acompanhamento de imagem. Exames de RNM atuais mostram aumento dos gliomas dos nervos ópticos.
Translation - English Throughout the diagnostic investigation, the patient was subjected to genetic testing in order to assess the possibility of neurofibromatosis type 1 (NF1), and the results showed an absence of pathogenic variants for genes NF1, NF2 and SPRED1. However, findings on the patient's brain MRI suggest NF1. Monitoring sessions with a geneticist revealed a ventricular tumoral lesion, which was then periodically monitored through radiologic studies. Serial testing results indicated a significant increase in lesion size in a short period of time, which prompted the patient's family members to seek neurosurgical assessment. Radiological findings evidenced an expansive lesion on the right frontal lobe, with an invasion of the anterior horn of the lateral ventricle closely approaching the foramina of Monro, but with no signs of hydrocephalus. Upon the evidence of lesion growth, with no defined cellularity diagnosis, it was decided that a biopsy guided by neuronavegation would take place, along with an endoscopic procedure with opening of the tuber cinereum, which was then performed. Anatomopathological findings were compatible with astrocytoma grade 1, a benign pathology. Due to the diagnosis of a benign pathology, and in light of the inherent risk of a lesion resection attempt, the final decision was to monitor the patient's condition through medical imaging. Recent MRI exams indicate growth in optic nerve gliomas.

Greetings! I am a native Brazilian Portuguese speaker living in the Netherlands. I've been speaking English since the age of 12 and recently got my C2 certificate.
