This would be my choice. "Personalized" and "personal" prescriptions are not one and the same. See refs.

I haven't see the term elsewhere, but it seems clear to me that this is the intent.

Well, if this is a condition, then presumably it isn't in the first person. I read שמי as שֵׁמִי.
